Risk / Incident / Problem Management

Reporting something that went wrong, or something that might. Injuries, near misses, breakdowns, hazards and problems - all of them start as a report here.

You only see what you have been given access to

ThreadWare is built from modules - Sales, Production, Quality and so on. An administrator gives you a role in each module, for each company you work in. The menu is built from those roles. So if something in this guide is not in your menu, it has not been given to you yet. Ask your administrator, or log a support ticket. To see what you currently hold, click your initials in the top right and choose My Access & Roles.

Can I use it?

Plumber (responder) A plumbing trade responder - sees plumbing incidents.
Risk Manager Manage the risks and incidents assigned to you, and produce analysis reports.
Risk Admin See and administer every risk and incident.

The two trade roles exist so that an electrical fault reaches an electrician and a water leak reaches a plumber, without either of them having to read everything.

Reporting something

  1. Open Report Risk / Incident / Problem.
  2. Choose the risk type. The list is maintained by your administrator and differs by company, so pick the closest match rather than a catch-all.
  3. Say where - the company and site.
  4. Describe what happened. Write it as if to somebody who was not there. What you saw, when, and what the effect was.
  5. Attach photographs if you have them.
  6. Set the severity, and who it should be assigned to if you know.
  7. Submit.
Report near misses, not just accidents

A near miss is the same event with luckier timing. Reporting it costs nothing and is the cheapest way your company will ever find out about a hazard.

Working an assigned item

Open My Assigned Risks / Incidents and click the row. From the detail screen you can:

Add an update every time something changes, even if the change is "waiting for the part". The history is what the analysis report is built from.

Reports and export

Risk Analysis Report gives you counts and trends by type, site and period. Most list screens also have an Export to Excel button on the toolbar.

Asking the assistant

Risks and incidents need no special role for the AI Assistant - any signed-in user can ask, and results are filtered to their own sites and access level. Try "what risks are overdue?" or "which risks have not been updated recently?".

When something looks wrong
